Penile Prosthesis Surgery
Malleable and Inflatable Penile Prosthesis Surgery for Erectile Dysfunction Treatment:
Penile prosthesis surgery is considered as the gold standard of treatment when other methods do not provide the desire results. From simple malleable implants to the most-advanced 3-piece inflatable devices, there is always one to choose from which would best fit your personal needs and financial availabilities,
Generally compared, the three-piece devices, e.g. the AMS 700 Series, are being considered as the most superior systems by providing a natural appearing erection and greater flaccidity than two-piece or malleable implants. These advanced, three-part prostheses are inflatable implants available in a variety of models and sizes for a custom fit. Each consists of a reservoir implanted in the abdomen, a pump placed in the scrotum and a pair of cylinders implanted in the penis. The entire device is totally concealed in the body.
The gold standard for innovative penile implant technology, the AMS 700 Series now features InhibiZone™ Antibiotic Surface Treatment and a special wear-reducing Parylene micro coating that increases the reliability and durability of the cylinders.
The AMS 700 penile prosthesis surgery is ideal for a man with good mental and physical dexterity, desiring a natural looking erection, and a more natural flaccid state, which is achieved with better results than those provided by one, or two-piece implants.
AMS 700 Series Advantages
- Acts and feels like a natural erection
- Expands the girth of the penis
- More firm and full than 1 or 2 piece implants
- Feels softer and more flaccid when deflated than 1 or 2 piece implants
- Totally concealed within the body
AMS 700 Series Disadvantages
- Requires some manual dexterity
- Possibility of leakage or malfunction
- Possibility of unintentional erections
- Possible penile shrinkage over time
2 Piece Series:
The 2- piece inflatable prosthesis consists of a pair of fluid-filled cylinders that are implanted in the penis and a single pump bulb that's implanted in the scrotum. The 2-piece prosthesis is ideal for a man who wants a rigid erection and easy deflation.
Malleable or 1 Piece Series:
The malleable or 1-piece prosthesis consists of a pair of cylinders that are surgically implanted into the penis. This non-inflatable penile prosthesis offers easy use, even for those with limited dexterity.
2 Piece Series Advantages
- Total concealment in body
- Simple use
- A squeeze-and-release pump to inflate for erection
- An easy-deflation mechanism
1 Piece Series Advantages
- Total concealment in the body
- Easy positioning in erect and concealed states
- Non-inflatable- no need for a pump to be implanted into the scrotum
Procedure Description:
Dr. Elist makes an incision just below the scrotum, in the perineum, or in the upper scrotum, at the base of the penis, to enter the erectile tissue.
With care to avoid important nerves and blood vessels, dissection is carried up to the erectile tissues at the base of the penis. Incisions are made in the thick fibrous membranes surrounding the two main erectile tissues to allow the insertion of dilators to create space for the prostheses. The paired prosthetic devices are inserted one at a time into the two erectile tissue compartments down the length of the penis. The incisions and tissues are closed by suturing.
The insertion of a three piece inflatable prosthesis, used for Erectile Dysfunction Treatment, follows the same principle. Dr. Elist inserts an inflatable penile prosthesis made up of three components; the reservoir, the pump, and two inflatable cylinders. A transverse incision is made at the base of the penis in the upper scrotum. With care to avoid the urethra and important nerves and blood vessels, dissection is carried down to the erectile tissues at the base of the penis. The thick fibrous membranes surrounding the two main erectile tissues are incised and dilators are inserted to create space for the prostheses. Two prosthetic devices are inserted into the two erectile tissue compartments down the length of the penis. A pouch is made in one side of the scrotum, and the pump mechanism and tubing are inserted into the space created. Using an index finger, Dr. Elist creates a tunnel from the pump to the space behind the pubic bone. The reservoir is placed behind the pubic bone with tubing running from it through the tunnel to the pump in the scrotum. The incisions and tissues are closed by suturing. This operation for Erectile Dysfunction Treatment can also be done in similar fashion using an incision just below the scrotum in the perineum to enter the erectile tissue and in the area above the pubic bone to gain access to place the reservoir.

Penile Shortening (A Complication of Penile Prosthesis Surgery):
Penile Prosthesis (used in Erectile Dysfunction Treatment) is an excellent solution for men who are suffering from treatment refractory erectile dysfunction and remain the last resort if Viagra, Cialis, etc., are not effective any more. As with all types of surgeries, the insertion of penile prosthesis as a solution for erectile dysfunction has its associated side effects and complications. While the majority of men report improved satisfaction and quality of life, penile shortening seems to be a very common complication of penis prosthesis surgery.
Penile shortening after prosthesis surgery (Erectile Dysfunction Treatment) seems to be caused by prolonged loss of erection which causes the penis to shrink, scar tissue formation due to surgery, and general fibrous tissue formation.
For as long as penile prosthesis has been used for erectile dysfunction treatment, men had to make a compromise between having erections and accept loss of penis size.
Solution for Penile Shortening After Penile Prosthesis Surgery:
Using his patented and 510(K) cleared subcutaneous soft silicone implant (ELIST Implant), Dr. Elist has helped many men with penile shortening after prosthesis surgery not to only regain their lost length, but gain additional length and girth.
For former patients who have had the penis prosthesis for their erectile dysfunction treatment, the enlargement surgery can be done immediately. If you are a current patient desiring a placement of penile prosthesis for erectile dysfunction and an enhancement surgery, please be advised that both surgeries cannot be performed in one setting. Penile prosthesis surgery (both, malleable and inflatable for erectile dysfunction treatment) must be performed first. After a recovery period of at least 6 weeks, the penile enhancement surgery can be performed if desired.

Scientific Solution to Retrieve Penile Size Loss:
In 2002, an 510K cleared by FDA method was developed by Dr. Elist, using a subcutaneous, soft silicone implant to permanently increase both penis girth and length simultaneously(Click to read more).
This technique was presented for the first time at the American Urology Association Western Division.
Furthermore, Dr. Elist’s revolutionary technique has been scientifically embraced by International Journal of Impotence Research to provide many men with their desired enhancement results. (Click for the article)
